Consultation Planning and Recording is a process for ensuring that patients get the most out of their medical visits. Parts of this process have been implemented at UCSF through the Decision Services initiative to promote patient participation and the UCSF Cancer Resource Center, as well as in community-based cancer resource centers around Northern California. Description of Consultation Planning and Recording

Patients about to consult specialists are often advised to “make a list of questions, bring a friend to take notes, and tape record your meeting.” Studies confirm that these actions can improve the quality of patient participation.

A Consultation Planning and Recording service provides trained navigators to support patients before, during, and after their visit to a medical specialist. Consultation Planners and Recorders document patient questions before an appointment. They accompany the patient to the visit, take notes, and audio-record the doctor’s advice and information. The patient leaves with a written summary of questions and answers, as well as the audio recording.
